Welcome to Dr. Hughes' website with patient portal.

In honor of the upcoming holidays, the office will be closed Thursday and Friday 11/24-11/25/16.
We also are closed Monday December 26, 2016 in Observance of the Christmas Holiday.
We will work 9-12 during the week between Christmas and New Years Day.
We resume normal hours Monday January 2, 2017.

We are proud to announce that we have achieved Level 3 recognition from the National Center for Quality Assurance, the highest level of recognition for Patient-Centered Medical Home.
We are very Proud of the work we are doing and look forward to providing excellence in healthcare going forward!

The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.

NCQA Patient-Centered Medical Home (PCMH) Recognition is the most widely-used way to transform primary care practices into medical homes.

 


We will be sending reminders for patients who have not had mammograms, pap smears, Bone Density Tests, Immunizations and other maintenance exams.
Please refer to the Patient Maintenance Screen for Specific Guidelines.

Here is what we have been up to, taking better care of YOU!!

Thomas Hughes, MD runs frequent reports in an effort to ensure quality care. These reports are meant to capture our patient populations who have not yet received various services, tests, screenings, vaccinations, etc. Our office staff places reminder calls, sends reminder letters, and utilizes the Patient Portal in an effort to provide quality care. We have included some of our outcomes. Please take a moment to review our results. We hope this information explains why you may have experienced an increase in outreach attempts from this practice.

Colonoscopy Outreach

In April 2015 there were 185 patients on our list who had not completed colonoscopy screening. Of these 185 patients, 100% received outreach from this office requesting they complete this screening. In August 2015 there were 98 patients on our list who had not completed colonoscopy screening. Of that 98, 100% received outreach from this office requesting they complete this screening.

The practice saw improvement in the completion rate in this area by 47%.

Mammogram Outreach

In April 2015 there were 67 patients on our list who had not completed mammography screening. Of these 67 patients, 100% of patients received outreach from this office requesting they complete screening. In November 2015 there were 145 patients on our list who had not completed mammography screening. Of that 145, 100% received outreach from this office requesting they complete screening.

The practice did not see improvement in the completion rate in this area; we are still working on improving our mammogram compliance numbers.

Chronic Care Outreach

Diabetes - In May 2015 there were 17 patients who had not completed lab work deemed necessary by our providers in the management of diabetes. Of these 17 patients, 100% received outreach from this office requesting they complete lab work. In September 2015 there were 12 patients who had not completed lab work deemed necessary by our providers in the management of diabetes. Of these 12 patients, 100% received outreach from this office requesting they complete lab work

The practice saw improvement in the completion rate of this area by 29%.

Chronic Obstructive Pulmonary Disease (COPD) - In September 2015 there were 140 patients who had not completed spirometry deemed necessary by our providers in the management of COPD. Of these 140 patients, 100% received outreach from this office requesting they complete lab work. In December 2015 there were 126 patients who had not completed spirometry deemed necessary by our providers in the management of COPD. Of these 126 patients, 100% received outreach from this office requesting they complete lab work

The practice saw improvement in the completion rate of this area by 10%.

Prevnar Outreach (pneumonia vaccination)

In June 2015 there were 233 patients on our list who had not received the pneumonia vaccination. Of these 233 patients, 100% received outreach from this office requesting they consider vaccination. In December 2015 there were 84 patients on our list who had not received the pneumonia vaccination. Of that 84, 100% received outreach.

The practice saw an improvement in the completion rate in this area by 64%.

Tdap Outreach (tetanus vaccination)

In June 2015 there were 508 patients on our list who had not received the pneumonia vaccination. Of these 508 patients, 100% received outreach from this office requesting they consider vaccination. In November 2015 there were 192 patients on our list who had not received the pneumonia vaccination. Of that 192, 100% received outreach.

The practice saw an improvement in the completion rate in this area by 62%.

Hospital Affiliation
Dr. Hughes currently has courtesy privileges at Eastern Niagara Hospital- Lockport

If you are admitted to Eastern Niagara Hospital-Lockport, The Hospitalist will be your provider.

While he does not admit to the hospital he does visit his patients everyday,when he is available. The Hospitalist will be your primary care giver during admission.

Please ask that ALL of your hospital records are faxed to Dr. Hughes at discharge.

Also, schedule a follow up within 1-2 weeks of discharge, and bring discharge paperwork and all of your medication with you.
